How do I cancel my conversion coverage?

If you are an employee of a corporate client with more than 5 employees: You can cancel your conversion coverage by notifying our office in writing that you wish to terminate coverage or by contacting the member service department at the number on the back of your ID card.
If you are an employee of a under 5-employee corporate client: You can cancel your conversion coverage by contacting the member service department at the number located on the back of your ID card.
