You can sign up for a group long term disability plan in one of two ways. 

1.    If you are an EMPLOYEE you may sign up through your employer if they offer a long long term disability plan; if your employer does not have one currently let them know you are interested.   

2.    If you are an EMPLOYER and are interested in a group long term disability plan please contact us so that we can get you started.
