What information is needed to get a group short term disability plan? |

We would need the following
FOR THE INITIAL QUOTE REQUEST:
1. A census form that has
all full-time employees. Go to option #4 of our "Getting Started" section to get started.
2. A copy of you most recent UCT-6 (or your state payroll form, if your state's form has a different code) for the insurance
carrier to verify the number of
employees enrolled and their full time or part-time status.
3. Their salaries since short term disability
is based on income.
TO ENROLL THE GROUP
1. All of the above, plus
2. A group
application;
3. Employee enrollment forms;
4. A check made out to the insurance carrier for the amount quoted.
In order to write a group short term disability
plan we may need to request additional information from you based on the
carrier you choose. However
the things mentioned before are common amongst all carriers.
